One of her care givers seems to think the arnica helps more than the others. The others haven't a clue. We're considering either PRP or Stem Cell Therapy but due to its expense and her being on a limited budget, we'd like to find something more affordable. Any recommendations would be appreciated.

If Arnica was helpful, you might try a topical pain reliever called StoPain roll-on available at Walgreens and Walmart or online suppliers. Another consideration is Salonpas patches also available at most stores.
At 90, she doesn't sound like a candidate for knee replacement surgery. One other consideration is contacting a "compounding pharmacy", explaining her situation and asking for recommendations for compounded topical pain relievers that will be the strongest topicals available.
