I think I know how you feel when you say you don't know where to start. I've been there. I know it's overwhelming.
Needless to say, your mom has a number of pressing issues to deal with all at once. So, hopefully, your mom's in a situation where she's both able and willing to make major changes and follow relatively strict protocols.
That said, if it were my mother, here's what I would be thinking:
First, for the breathing: Bill Munro's hydrogen peroxide inhalation therapy. You can do a search for it on this site.
Second, the constipation... Iodine (with supplemental minerals including organic selenium, magnesium, and zinc) and dietary changes (dramatically reducing or eliminating sugar and near sugars, eliminating meat and dairy, and consuming lots of good fats, especially coconut oil). I strongly encourage you to research iodine for yourself by watching and learning from a few excellent YouTube videos by Dr. Brownstein and Dr. Group.
Third, digestion. Pork pancreas (digestive enzymes) and HCL (hydrochloric acid)... Unless, God forbid, she has some very serious gastrointestinal disease, this will help her digestive issues and help her begin to get some much needed nutrition.
Fourth, the UTI... Borax: Vaginal insertion (in a large capsule) and drunk dissolved in distilled or otherwise purified water as described on this site and as Dr. Walter Last describes it in "The Borax Conspiracy" and "The Ultimate Microbial Cleanse":

Fifth, Vitamin D and Vitamin C to aid the absorption of everything else and Ubiquinol, a good chromium supplement (like chromium polynicotinate), and R-Lipoic acid for the diabetes and associated complications.
Sixth, get her lymphatic system flowing any way you can -- standing with her on a rebounder if she can do it, walking if she can't do that, deep breathing if she can do neither, or lymphatic massage and skin brushing if she can do none of the preceding.
Seventh, detox. Iodine and Borax and Pork Pancreas and Vitamin C and lymphatic work should all assist with the detox.
Eighth, reduce the inflow of new toxic challenges by as much as possible dramatically reducing or eliminating sugar, dairy, and meat, all canned food, drugs (both prescription and non-prescription) and plastic food containers and protecting her as much as possible from fluoride and chlorine by eliminating fluoride toothpaste and getting good water (preferably) and air filters.
Incidentally, it's just a fact of life that detox symptoms usually accompany both therapies and most other healing protocols. Be prepared for them by educating yourself about them and doing things to minimize them, but don't let them dissuade you from your healing path. In all likelihood, the worse they are, the more your mom needs them.
By the way, your mom's nail coming off (along with various other aspects of her condition) make me suspect fungal involvement. And the iodine, the borax, and the dietary changes should all go a long way in helping you/her resolve that challenge.
One more thing... I don't envy you the challenge of taking care of a family member with various pressing challenges at the same time that you need to get up to speed about various therapies. However, to the maximum degree possible, spend every possible second you can studying these therapies so that you can minimize the inevitable mistakes and correct them as quickly as possible.
