Perhaps using just water with the hydrogen peroxide is too harsh for your father.
In the link to “Spirit of Change” below, under the section “Nebulized Peroxide Basics” (about half way down the page), Dr. Mercola gives you the formula for nebulizing.

In addition to all of the things you are doing, you might want to run a cool mist ultrasonic humidifier with distilled water and hydrogen peroxide 24/7. I find I breathe easier with it running – especially now that windows are usually closed due to the cold weather.
My humidifier holds 1 gallon of water (I use distilled). I add 1-1/2 cups of 3% hydrogen peroxide to it then fill the remainder with distilled water.

Also, you might want to get a pulse oximeter (they are quite cheap) to put on your dad's finger to monitor his oxygen level and pulse.
