Dont know what to do for pain in neck and shoulder

I don't know what to do. My neck and shoulders are driving me nuts. It feel like something is in there irritating and hurting me. I have lymes and arthritis. I am utterly exhausted and have lots of gastric distress. My sleep is very bad.
phys. ther.acu, guasha, massage chiro and supplements have not helped and I feel desperate.
