Studies using nattokinase have shown that daily dosing of higher fibrinolytic units (FUs), which are a measurement to describe the potency of Nattokinase, show that it can gradually reduce atherosclerosis over a period of about one year or more in humans. It has been shown that nattokinase can dissolve blood clots in animal studies and it is suggested that it may act similarly in humans, but the prescription anticlotting agents are significantly more potent and may be a better choice for treating existing blood clots quickly, whereas nattokinase can potentially dissolve blood clots at a slower pace over a longer period of time.
It is just my opinion, but considering the potential risks of blood clots, it seems like it may be a safer option to use prescription anticlotting agents under your doctor's supervision to try to dissolve them as quickly as is reasonably possible. There are still risks involved either way.
In the case of blood clots, I would tend to look at nattokinase at higher FUs as a potential inhibitor of blood clot formation and not so much as a treatment to dissolve existing blood clots. More to prevent them from forming in the first place or to help prevent them from forming again once you are rid of the existing clots.
I wrote about the use of nattokinase for atherosclerosis here :
